Job Description
Eventide's Home Health Care in Grand Island, NE is seeking a Physical Therapist to work full time 8a- 4:30p. The Physical Therapist is responsible for providing Physical Therapy services to Eventide’s Home Health Care Seniors/residents/clients with physical or cognitive disabilities to maximize functional independence.

Requirements

  • Evaluates clients utilizing appropriate diagnostic muscle, nerve, joint and functional ability tests.
  • Develops and implements the client's plan of care in collaboration with the physician, other consulting health care professionals, family/caregiver according to agency procedures and state and federal regulations.
  • Communicates and coordinates care and client's progress with other team members involved in the care.
  • Ensures that assigned client’s clinical records are complete, timely, and reflect coordination of care.
  • Oversees and coordinates Physical Therapist Assistant and Rehabilitation Aide client care activities.
