Role OverviewProvide evaluative, consultative and treatment interventions to home health patients in their place of residence. Collaborate with interdisciplinary team and attend physician to develop and manage patient care plan.
What You Will Do
Evaluate patient's level of function and skilled therapy needs, treat patients to relieve pain and develop or restore function, and advise and consult with family and other team members.
Why It Might Be a Fit
Must have a valid professional license in State of practice, graduate of an accredited Physical Therapy Program, and prior Home Health experience is preferable.
