Physical Therapist Assistant - Home Health - Full Time - Kearney/Buffalo County and Surrounding Area

Requirements

• Reviews senior’s/client’s chart and obtains current information that may affect Physical Therapist treatment
• Recognizes abnormal physiologic changes and reports them to the supervising Physical Therapist
• Performs Physical Therapy treatment procedures as delegated by supervising Physical Therapist in accordance with the initial evaluation and plan of care
• Develops and implements the Senior’s/client’s plan of care in collaboration with the physician, other consulting health care professionals, and the family/caregiver
• Actively participates in senior/client care conferences and provides input to the Physical Therapist for developing the plan of care
• Actively maintains work schedule which allows for coverage in all Rehab programs as needed
• Uses strength, agility, stamina and prioritization skills to respond promptly to frequent and often overlapping senior/client/caregiver/TEAMember/manager needs or requests
• Maintains written documentation reflective of the department standard
• Communicates effectively with families/caregivers, TEAMembers, and support services to achieve Senior/client goals and meet service expectations
