Role OverviewWe are seeking an experienced Physical Therapist to perform home health visits for our senior living office. The Physical Therapist will be an integral member of a multi-disciplinary health care team providing rehabilitative care and skilled nursing to clients.
What You Will Do
Make home visits to clients, perform diagnostic tests, implement care plans, and educate clients and family members on rehabilitative care.

Requirements
- Current Pennsylvania Physical Therapist license
- Minimum of one year of recent work experience as a Physical Therapist
- Graduation from a program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association
- Ability to work independently and manage time effectively
- Strong interpersonal, organizational, and problem-solving skills
- Solid computer skills; prior experience with electronic medical records (EMR) preferred
