Requirements

• Evaluate patient's level of function and skilled therapy needs
• Treat patient to relieve pain, develop or restore function, and obtain maximum outcomes
• Advise and consult family and other interdisciplinary team members
• Assess, monitor, document and report progress of patient's health and condition
• Collaborate with interdisciplinary team and attending physician to develop, integrate, and manage patient care plan
• Conduct field visits to evaluate and initiate start of care and conduct clinical outcomes monitoring
• Utilize accepted company teaching materials in patient/family education
• Complete documentation timely, accurately, and at the point of care
• Meet agency productivity standards and utilize time and resources effectively
• Participate in center/agency surveys, mandatory in-services, case conferences, staff meetings, and programs
