Job Description
Physical Therapist Assistant who provides exceptional therapy to patients in their own home, working autonomously with variety in their day and ample time for 1:1 patient care. Requirements

  • Implements physical therapy treatment plans in consultation with supervising physical therapist.
  • Reinforces therapeutic plan following discharge by instructing patients, families, and caregivers in home exercise programs.
  • Communicates need for changes in patients goals to supervising physical therapist.
  • Effectively communicates all aspects of patient care to appropriate team members; contacting team manager as needed.
  • Communicates patient’s progress or need for discharge to supervising physical therapist.
  • Takes lead role in educating patient and caregivers on proper body mechanics.
  • Documents in patient electronic records by assigned deadlines.
