Requirements

  • Provider is responsible for rounding and managing clinical care of population at assigned SNF
  • Provider is responsible for daily patient rounding and prescribing medications/treatments for SNF residents
  • Provider will be available for phone/secure messaging consultations with facility nursing staff
  • Provider documentation must be completed in ChartEasyTM EHR system
  • Provider is expected to be on-site for scheduled shifts, with timely attendance
