Job Description
Provide full scope of services in Family Practice or Internal Medicine, including diagnosis, treatment, and coordination of care, while following quality management and customer service programs.

Requirements

  • Doctor Osteopathy (D.O.) or Doctor of Medicine (M.D.)
  • Completed residency in primary care specialty and Board Certification
  • Minimum two (2) years of working experience in primary care medicine in an outpatient setting
  • Current licensure in the State of Illinois, DEA, Controlled Substance and maintain CME requirements
