Role OverviewThe PCAN Navigator will outreach, educate, and facilitate the enrollment of uninsured patients into PCAN 'medical homes.' The role involves administrative and casework duties, including interviewing applicants, determining eligibility, and linking community residents with service agencies.
What You Will Do
Provide outreach/education, interview potential clients, determine eligibility, complete paperwork, and follow up with clients to determine outcomes of enrollment process.
Why It Might Be a Fit

Requirements
- Associates Degree from an accredited institution of higher learning
- Two (2) years of experience in social, health, or public services, or an equivalent combination of relevant education, training, and experience
