Requirements

  • Doctor of Medicine (MD) or Osteopathic (DO)
  • Board certified or board eligible in Family Medicine or Internal Medicine - Geriatrics specialty
  • Valid, unrestricted license (or willing to obtain) in the state of desired employment (Florida)
  • Current, unrestricted DEA license
