What You Will Do
Conduct thorough in-person assessments and provide high-touch primary care. Lead Interdisciplinary Team (IDT) meetings to craft and execute personalized, holistic care plans. Oversee transitions of care, including acute episodes and long-term placement management.

Requirements
- Completion of an accredited residency in Internal Medicine, Family Medicine, Emergency Medicine, or Geriatrics
- Active, unrestricted state physician license
- CDS certification
- DEA registration
- Board Certification is preferred
- Ability to maintain CPR and First Aid certification
- 2–5 years of primary care experience is ideal, but passionate new graduates are encouraged to apply
- Eligibility to serve Medicare and Medi-Cal participants
