Role OverviewAs a Home Health Social Worker, you will be responsible for assessing the psychological status of patients, carrying out social evaluations, and providing information and services to patients and families. You will be part of a team that provides high-quality care to help patients maintain their independence and dignity in the comfort of their own homes.
What You Will Do
Your primary responsibilities will include assessing the psychological status of patients, carrying out social evaluations, maintaining clinical records, serving as a liaison between patients or families and community agencies, and participating in the development of the total plan of care and case conferences.

Requirements
- Master of Social Work (MSW) degree or a Baccalaureate degree (BSW) from an institution accredited by the Council on Social Work Education
- current CPR certification
- licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order
- at least 1 year of social work experience in a healthcare setting (preferred)
- current Medical Social Worker Licensure in the states of Ohio and West Virginia
