Performs evaluation(s) and develops an effective treatment plan, educates patient/family/caregiver, and initiates discharge planning process.

Requirements
- Graduate of an accredited Physical Therapy program
- Licensed Physical Therapist, Ohio and/or Multi State Compact license
- Basic Life Support (BLS) Required within 90 Days
