Role OverviewProvide evaluative, consultative, and treatment interventions to home health patients referred by a licensed physician in their place of residence. Maintain effective communication with Home Health partners, United Rehab partners, patients, patients' families, and other internal and external customers as appropriate.
What You Will Do
Complete evaluations, assessments, and treatments in accordance with policy and the individualized care plan and within established timelines. Helps maintain continuity of care by collaborating with the interdisciplinary team and attending physician to develop, integrate, and manage the patient care plan of care, making referrals to other disciplines as needed.

Requirements
- Must have a valid professional license in state of practice.
- Graduate of ASHA accredited school.
- Prior Home Health experience is preferable.
