Job Description
The Transitional Care Coordinator collaborates with the patient's care team to enhance the patient experience and prepare the patient for the next site of care. The TCC develops and implements a patient's post-acute care plan, identifies barriers to follow-up care, and facilitates necessary referrals.

Requirements

  • Graduate of an accredited school of nursing
  • 1+ years clinical nursing experience (acute care)
  • Knowledge of Medicare, Medicaid and commercial payer regulations
  • Computer competency
  • Multi-tasks and prioritizes work
  • Works autonomously
  • Communicates effectively with persons of various backgrounds
  • Maintains clinical competency regarding health care delivery/regulatory requirements
  • Teaching skills
  • Registered Nurse (RN), Ohio and/or Multi State Compact License
