Job Description
The Program Assistant works with the Assertive Community Team to provide care and treatment for clients with significant mental health/psychiatric symptoms. The role involves phone support, crisis assessment, and coordination of care with team members and outside providers.

Requirements

  • Provides interventions or support services as outlined by the Individual Crisis/Coaching Support Plan
  • Coordinates with the ACT team and other community resources
  • Treats clients in a respectful manner and conducts services with regard to client dignity and self-determination
  • Completes general office duties
  • Maintains service standards
  • Achieves client specific and program outcomes
