Job Description

We are looking for staff to provide direct care to persons with developmental and physical disabilities in our Residential Group Home. This home is located in Terryville, a mere 15 minutes from Waterbury. This person will be responsible for implementing behavior programs and other habilitation programs, and also facilitating and promoting community integration and safety.

Requirements

Ensure individuals’ medical health issues are addressed.
Assist individuals in daily activities including daily hygiene functions.
Implement behavioral goals, teaching strategies, monitoring procedures, exercise programs and other team recommended programs and procedures.
Assist in the completion of individual assessments.
Model positive adult roles in the community
Foster community belonging through person-centered appropriate activities
Supervises and assists in the preparation of food for individuals following special prescribed diets and food consistency. Implements eating procedures as written, if applicable.
Overnight awake staff is required to complete hourly safety checks on the individual
Assist individuals in community integration and improving their overall quality of life.
