Role OverviewThe Program Manager, SDOH will contribute to the well-being of the community by enhancing programs that address vital social determinants. This role is part of a mission-driven environment where work directly impacts the health and happiness of the community.
What You Will Do
Key responsibilities include communicating with sites and training staff on workflows for the Food Insecurity Initiative, maintaining and distributing the FI Resource Guide to patients, and developing and revising training curriculum related to Social Determinants of Health.

Requirements
- BSW or MSW from a CSWE accredited institution required
- Minimum of 2 years experience in social services in a clinic setting required
- LMSW licensure with 2+ years of direct practice experience preferred
- LCSW preferred
- Excellent interpersonal and communication skills, both verbal and written required
- Good knowledge of Microsoft Office suite
- Knowledge of public benefits and community resources
