Role Overview

Personalized Health Partners is recruiting a Psychiatric Mental Health Nurse Practitioner (PMHNP) to join the team at Brookside Healthcare Center in Cincinnati, OH. This full-time role involves assessing, diagnosing, and treating residents in a long-term care facility setting, serving as a leader on a multidisciplinary team to enhance resident well-being.

What You Will Do

Conduct thorough behavioral health assessments and evaluations for residents. Develop and oversee individualized treatment plans in collaboration with residents, families, and interdisciplinary teams. Prescribe and manage appropriate medications and therapies, provide counseling and support, monitor resident progress, and educate residents and staff about behavioral health conditions and resources.

Requirements

  • Master's or Doctorate Degree in Nursing with a specialization in Psychiatric nursing
  • Graduation from a state-certified advance practice program specializing in psychiatry/mental health
  • At least 1-2 years of direct clinic care experience in behavioral health
  • Proficiency in working with both adult and geriatric populations
  • Specialty NP experience in Behavioral Health
  • Strong interpersonal and communication skills
  • Previous experience in psychiatric nursing before transitioning to NP
  • Dedication to evidence-based practice and continuous professional development
  • Must have a certification by the American Nurses Credentialing Center (ANCC) in Psychiatric Mental Health (PMH) as a Psychiatric Mental Health Nurse Practitioner (PMHNP)
  • Must have an unrestricted license to practice as an advanced practice registered nurse (APRN) in the state of Ohio
  • Must have a valid DEA number and unrestricted DEA license in Ohio
  • Must have a state-issued Controlled Substance Registration (CSR) in Ohio
  • Must be registered with the state Prescription Monitoring Program (PMP) in Ohio where direct services will be provided
  • No suspension/exclusion/debarment from participation in federal health care programs (e.g. Medicare, Medicaid, SCHIP)
  • Maintain credentials and licensure with all major insurance companies and licensing boards with the assistance of Provident credentialing staff
  • Experience in delivering high-quality, patient-centered care
  • Proficiency in documenting and navigating electronic health records (EHR) independently
  • Ability to maintain strict client confidentiality
  • Ability to serve as a representative of PHP, displaying courtesy, tact, consideration and discretion in all interactions with team members
  • Proficient computer skills, including working knowledge of Microsoft Office Suite
  • Ability to work effectively on multidisciplinary team
  • Ability to read, analyze and interpret general and professional documents
  • Ability to write routine reports and correspondence
  • Ability to effectively present information in one on one and small group situations
  • Must possess the ability to make independent decisions when circumstances warrant such action
  • Ability to foster teamwork; train and direct the work of team members and colleagues
