Requirements

  • Assesses and attends the psychosocial needs of patients and their families
  • Provides evaluations, ongoing monitoring and individual and group counseling
  • Maintains continuity of care by developing and implementing patient discharge plans
  • Acts as liaison between facility, family, government and community agencies
