Job Description
Qualified Developmental Disability Professional responsible for coordinating Individual Support Team functions and responsibilities, planning and scheduling meetings, and assisting individuals to lead their meetings. The role requires developing individual program plans, reviewing effectiveness of plans, and ensuring people have annual physical exams and accurate face sheets and contact information.

Requirements

  • Bachelor's Degree in a human services field (Psychology, Sociology, Special Education, Social Work, or a related field)
  • Registered Nurse with a minimum of one year experience working with individuals with developmental disabilities
  • Ability to interpret and implement support plans and to motivate, instruct, and supervise persons with intellectual disabilities
  • Knowledge of psychiatric disorders and intellectual disabilities in the adult population
  • Knowledge of cultural diversity
  • Knowledge of available resources
  • Knowledge of behavior management, crisis intervention, and interventions common to individuals with intellectual disabilities
  • Knowledge of non-violent crisis intervention practice
  • Knowledge of all applicable standards (DMH, Joint Commission, Medicaid)
  • Knowledge of Person Center Planning, natural supports, and the philosophy of self-determination
  • Knowledge of utilizing electronic incident reporting
