Re-Entry Specialist (REENTSPC-08) Mon-Fri 11am-8pm

Requirements

• Case management delivery
• Excellent communication skills
• Ability to partner with members to develop a role for themselves outside of recovery and re-entry
• Ability to provide supportive services including assistance in maintaining, monitoring, and modifying covered services and case management plans
• Ability to identify necessary resources to meet members’ needs and help members with obtaining needed resources
• Ability to manage discharge plans upon completion of treatment
• Ability to act as the member’s advocate as it relates to insurance coverage and financial assistance
• Ability to maintain comprehensive case management records and notes through detailed, accurate, and timely documentation
• Ability to facilitate placement programs and provide resources
• Ability to adjust to fluctuating peaks in member flow, acuity, and other operational demands
• Ability to collaborate professionally with courtesy and tact
• Ability to transport members to their desired destinations
• Ability to assist with monitored urinalysis/drug testing of members
• Ability to meet Key Performance Indicators (KPIs)
• Ability to receive and actively engage in clinical supervision and oversight each week/month
