Role OverviewThe Referral Center RN is responsible for receiving and processing referrals for seamless transitions of care and providing information to the public regarding agency service line. The role involves comprehensive clinical assessment/screening of patients, collecting and documenting detailed information, and working collaboratively with the healthcare team to ensure quality of services and patient safety.
What You Will Do
The RN will obtain required clinical information, process MD orders, expedite requests for services, and maintain knowledge of current agency services and criteria for admission. They will also communicate effectively on e-discharge systems, efficiently enter data into the EMR, and complete Insurance Verification process for referrals entered.
Why It Might Be a Fit
The ideal candidate will have 2 years' experience in healthcare, homecare experience preferred, and experience with EMR and E-Discharge platforms. They will be able to multi-task and manage multiple aspects of the referral process.
Requirements
- RN currently licensed in Massachusetts (job code RNCT/RNCTPD) or LPN currently licensed in Massachusetts (job code 10611)
- 2 years' experience in healthcare, homecare experience preferred
- Experience with EMR and E-Discharge platforms preferred
