Role OverviewJoin Avance Care's rapidly expanding network of 37 practice locations in the Triangle Area, Charlotte Region, and Wilmington, NC. As a Referral Coordinator, you'll aid providers with continuity of care by ensuring referrals get submitted promptly to high-value specialists and close the loop of care by obtaining consult notes after specialist visits.
What You Will Do
Ensure referrals are submitted, accepted, and followed up on in a timely manner, provide clinical information to outside entities, maintain detailed and accurate referral documentation, and establish relationships with providers and outside entities.
