Job Description
Easterseals PORT Health is seeking an empathetic and dedicated Referral and Entry Coordinator to support and connect individuals with needed resources as a part of their VA REACH team in Northern Virginia. The role involves evaluating eligibility, processing referrals, and sharing program information with community partners.

Requirements

  • Master’s in health, psychology or social work
  • Licensure: Licensed or Licensed-Eligible as a LCSW, LPC, LMFT
  • Skills: Crisis intervention, adaptability, problem-solving and decision making
  • Experience: Working with the I/DD population, including programming for individuals with significant behavioral needs.
  • Maintain a valid driver's license, good driving record and current auto insurance.
