Job Description
Director of Rehabilitation Services position at Taconic at Hopewell, a premier skilled nursing and rehabilitation center, seeking a licensed therapy professional to lead and elevate the therapy department.
Requirements
Licensed PT, OT, or SLP
Minimum 3–5 years of rehabilitation experience in skilled nursing or post-acute care
Prior leadership or supervisory experience preferred
Strong knowledge of Medicare, PDPM, and regulatory compliance
Proven ability to improve clinical outcomes and team performance
Excellent communication, leadership, and organizational skills
