Rehousing Case Manager

The Rehousing Case Manager provides housing-focused case management for shelter guests, providing access to resources as needed, completing required assessments and paperwork, and ensuring each guest has a goal plan and identified path towards housing. Case managers operate from a housing-first approach, beginning to exit planning process at their first meeting with guests and using creative support and interventions to help move guests from homelessness to housing as quickly as possible.

What You Will Do

Schedule and meet with new guests within 72 hours of their arrival in shelter, complete all required paperwork and assessments, collaborate with Rehousing Manager, Housing Navigator, Continued Care Staff, and Behavioral Health and Wellness team members to assist guests in growing stability, and provide appropriate resources and support for guests.

Requirements

• Schedule and meet with new guests within 72 hours of their arrival in shelter
• Meet with guests a minimum of twice monthly
• Complete all required paperwork and assessments
• Collaborate with Rehousing Manager, Housing Navigator, Continued Care Staff, and Behavioral Health and Wellness team members
• Provide appropriate resources and support for guests
• Assist guests with benefits applications, updates, and navigation
• Attend weekly team meetings, quarterly all-staff meetings, and any additional mandatory trainings
• Ensure all guests receive a warm handoff to housing navigation and again into continued care when they progress along their housing journey
• Have a valid driver's license and pass a motor vehicles record check
