Role OverviewThe Reintegration Case Manager will support adults transitioning from incarceration back into their communities through stable housing, employment, behavioral health treatment, recovery supports, healthy relationships, and community connection. This position is ideal for someone who enjoys building relationships, empowering others, and making a direct impact in the lives of individuals working toward stability, independence, and positive community reintegration.
What You Will Do
Provide Case Management and/or CBRS services to adults with mental health, behavioral health, and reintegration needs. Complete assessments, assist clients in identifying strengths, barriers, goals, and support needs, and develop individualized service plans.

Requirements
- Bachelor's degree in Social Work, Psychology, Counseling, Criminal Justice, Human Services, or a related field
- Relevant experience may be considered in lieu of degree requirements where applicable
- Experience working with mental health, substance use, justice-involved populations, or community-based services preferred
- Strong communication, organization, and problem-solving skills
- Ability to build rapport while maintaining professional boundaries
- Ability to work independently and collaboratively within a team environment
- Valid driver's license, reliable transportation, and ability to travel throughout the service area
