Job Description
We are looking for an RN to deliver patient centered health coaching and chronic disease management via chat, phone, and video for a company that specializes in cardiometabolic care, with a salary of up to $100,000.
Requirements
Strong empathy, communication, and patient-centered care skills
Bachelor of Science in Nursing (BSN) required
Active Registered Nurse license in good standing, with ability to obtain additional licensure if needed
Certified Diabetes Care and Education Specialist (CDCES) or willing to pursue if not currently certified
3+ years of direct patient care in an ambulatory or telehealth setting
Adaptable, self directed, and comfortable working independently
Proficiency with digital tools for care delivery
Flexibility to participate in rotational weekend or holiday coverage
