Role OverviewThe Residential Case Manager provides clinical case management services to support and advance program goals and objectives through a high-quality, client-centered approach. They identify clients’ needs using a person-centered framework that addresses mental health symptoms as well as co-occurring disorders. They collaborate with clients to achieve the highest possible level of independence.
What You Will Do
The Residential Case Manager coordinates and delivers person-centered, recovery-oriented case management as part of a multidisciplinary team. They work closely with the care team to align case management services with clients’ mental health treatment plans. They conduct comprehensive assessments to identify client needs and develop individualized care plans.

Requirements
- High School or GED Required
- Minimum 1-to-3 Years of experience in a behavioral health setting
- Experience in residential facilities preferred
- Valid driver's license, registration, current inspection and insured automobile
- Manual dexterity and mobility required
- Strong knowledge of community resources and systems
- Ability to build rapport and establish trust with diverse clients and community partners
- Cultural competence and commitment to providing equitable and inclusive services
- Ability to communicate effectively, both verbally and in writing
- Excellent organizational, problem-solving, and time management skills
- Proficiency in documentation and electronic record-keeping systems
