Job Description
We are seeking a Resource Consultant to guide, mentor, and support individuals with disabilities and the elderly in self-directing their Personal Options services through the West Virginia waiver programs. The successful candidate will have experience working with the public, preferably in patient navigation, community connection, or case management, and will be able to demonstrate expertise in the Personal Options services program and Palco policies.

Requirements

  • Meet with participants in their homes at least every six (6) months
  • Demonstrate expertise of the Personal Options services program and Palco policies
  • Educate and mentor participants on their rights, responsibilities, and risks in the program
  • Monitor and evaluate participants on their ability to participate in the program
  • Assist members and representatives in developing and implementing their service/spending plan and subsequent updates
  • Thoroughly and accurately document visits with participants
  • Perform required administrative tasks daily
  • Follow operational procedures as defined by Palco
