Role Overview

The Resource Specialist is a member of an integrated primary care and behavioral health care team in adult medicine that connects patients to social resources to support their health and wellness.

What You Will Do

Meet with and outreach to patients to determine their social determinants of health needs, provide referrals to government- and community-based organizations, educate patients on healthcare and social services available to them, and assist patients in completing necessary referral forms.

Requirements

  • High school diploma or equivalent
  • Associate Degree or Bachelor's degree in health sciences, community health, social work, psychology, public health
  • Community Health Worker certification
  • Experience working in a health care setting
  • Medical terminology knowledge
  • Ability to manage multiple patients and diverse tasks
  • Ability to utilize EMR and Microsoft Office software
