Role OverviewAs a RN - Case Manager, you will provide skilled visits to patients in their homes, addressing their full scope of needs and helping them overcome real challenges. You will work with a manageable caseload and have the autonomy to make clinical decisions independently. What You Will Do
Your main responsibilities will include performing tasks involving physical activity, such as heavy lifting, bending, and standing. You will also be required to successfully complete a background check and have a current driver's license, vehicle insurance, and/or access to dependable transportation.

Requirements
- Current license with the state Board of Nursing
- 1 year of nursing experience is required - home care experience preferred
- Ability to perform tasks involving physical activity, which may include heavy lifting up to 50 lbs, bending, and standing
- Current Driver's License, vehicle insurance, and/or access to dependable transportation
- Ability to successfully complete a background check
