Community Care is hiring a RN Case Manager to join our Family Care Program team. Requirements

  • Associates degree in Nursing Required, Bachelors Degree preferred
  • Wisconsin Licensed Registered Nurse
  • Eligibility for long-term care functional screener certification is required
  • WI Drivers License
  • Required 2 years recent experience with older adults or persons with physical or intellectual disabilities in a nursing capacity Required
  • Preferred 2 years of work in community nursing
