Role OverviewCoordinates care for groups of patients, formulates and facilitates discharge planning, and monitors resource utilization. Utilizes nursing process and critical thinking skills to coordinate care. Serves as an educator on the Care Management process.
What You Will Do
Collects and aggregates data, identifies problematic discharges, and attends unit-based problematic discharge meetings. Identifies potential risk and intervenes with system inefficiencies.

Requirements
- Bachelor's Degree Graduate of an accredited school of nursing
- 2 years of relevant experience in the acute care setting
- 3 years of relevant experience in care management, utilization review, home care, and/or discharge planning
- Registered Nurse (RN) - State of Michigan
- Basic Life Support (BLS) - AHA American Heart Association or ARC American Red Cross
