Job Description
Community Care is offering a $8,000 Sign-on Bonus for a Full-Time RN Care/Case Manager to join the Day Shift in Kenosha County. The RN Care/Case Manager will collaborate with the site management team to establish and maintain uniform, consistent care delivery for Community Care members.

Requirements

  • Associate degree in nursing required.
  • Minimum 2 years of personal, clinical leadership, and or employment experience providing care or service for a frail or elderly population required;
  • 2 years of recent case management preferred.
  • A current Wisconsin Registered Nurse license in good standing
