Role OverviewThe RN Care Manager Ambulatory plans and prioritizes care for individuals and populations of patients, focusing on strategies that promote optimal health within populations. They assess internal and external referrals to identify patient/significant others' needs and develop a plan of care. The RN Care Manager Ambulatory communicates and collaborates with patients, providers, and payers to coordinate services and promote optimal health in a cost-effective manner.

Requirements
- Bachelor's Degree in Nursing
- State of Michigan RN License
- 5 years of relevant experience clinical experience preferred
- 2 years of relevant experience of clinical nursing and or case management/managed care or related field Required
