Role OverviewPlans and prioritizes care for individuals and populations of patients, focusing on strategies that promote optimal health within populations. Demonstrates expertise in nursing care and management of a caseload of patients of varying complexity.
What You Will Do
Develops plan of care, makes recommendations to PCPs and specialists, assesses educational needs of patients and families, communicates with patient/significant others/providers/payers to coordinate services, and measures patient care outcomes.

Requires 2-5 years of relevant experience in clinical nursing and/or case management/managed care, and a Bachelor's Degree in Nursing or a healthcare-related field. Preferred certifications include CRT-Case Manager, Certified (CCM) and Registered Nurse (RN) - State of Michigan.
