Job Description
Cheer Home Care is looking for a RN Care Navigator who loves spending time with older adults and provides high quality care in home. The RN Care Navigator collaborates with interdisciplinary team members to develop and implement individualized care plans and provides constant feedback to clients, client representatives, caregivers and nurses.

Requirements

  • Active California RN License
  • Master in Nursing (preferred)
  • Industry Experience (preferred)
  • Strong Computer Skills
  • Ability to multitask, drive, take action
  • Strong Organizational & Communication Skills
