Role OverviewThe RN Case Manager assesses the physical, functional, social, psychological, environment and financial needs of clients/patients; identifies a cost-effective comprehensive plan to meet the families' service needs, and implements the plan. Reports directly to Director of Clinical Case Management.
What You Will Do
Provide referrals to appropriate community resources, facilitate access and communication when multiple services are involved, monitor activities to ensure that services are actually delivered and meet the needs of clients, coordinate services to avoid duplication, and act as patient advocate.

Requirements
- Graduate of an approved School of Nursing, BSN preferred, Case Management certification preferred
- Minimum of three years med/surg experience in an acute care hospital
- Computer skills at data entry level
- Current Louisiana RN License or Compact State License and CPR certified
- Knowledge of community resources and programs
