Role OverviewThe RN Case Manager is responsible for the clinical management and coordination of skilled nursing services for patients admitted to Oswego Health Home Care. This role includes conducting comprehensive assessments, developing and updating individualized plans of care, and overseeing the delivery of skilled services in accordance with provider orders.
What You Will Do
The RN Case Manager provides ongoing evaluation of the patient status, manages care transitions, and collaborates with the interdisciplinary team to ensure quality outcomes, regulatory compliance, and continuity of care.

Requirements
- Licensed professional nurse
- Minimum of 2 years' clinical experience in a hospital setting
- Utilization review, clinical record, and/or managed case experience desired
- BLS/CPR certification
- Microsoft Word, Microsoft 365, electronic health records, and clinical documentation skills
