Job Description
We are seeking a top-tier RN Case Manager to join a mission-driven hospice and palliative care organization. As an RN Case Manager, you will provide support and expertise to patients in their homes, assisted living facilities, or skilled nursing homes.

Requirements

  • Current Georgia RN license
  • Valid drivers license, reliable transportation, and automobile insurance
  • At least 1 year of clinical nursing experience (hospice or home health experience is a plus!)
  • Strong communication skills and a commitment to patient-centered care
  • Ability to manage complex cases with confidence and empathy
