Role OverviewTennessee Quality Care Hospice is seeking an RN Case Manager to join their growing team in Jackson. This field-based position is responsible for managing a caseload of patients, providing therapeutic nursing, coordinating care, and supporting patients and their families in the comfort of their homes.
What You Will Do
Assume primary responsibility for patient and family caseload, including assessment, determining eligibility, admission processing, planning, implementation, and evaluation phases of the nursing process. Provide regular visits to re-assess the patient's needs and update the plan of care.

Requirements
- Graduate from an accredited registered nursing program
- Licensed as a registered nurse in the state of practice
- Current CPR, First Aid Certification
- Knowledgeable of quality assessment and assurance procedures
- Valid driver's license and proof of insurance
