Job Description
Community Healthcare of Texas has provided Hospice and Palliative Care Services since 1996. The RN Case Manager will meet with patients and families to explain the scope of hospice services, coordinate admission of patients, and ongoing plan of care.

Requirements

  • Registered Nurse in the state of Texas
  • Hold a valid driver’s license and have a dependable means of transportation
  • 1 year of nursing experience
  • Case Management experience (hospice)
  • 2+ years of experience in hospice case management strongly preferred
  • Proficient skills using a computer are required; Microsoft Office suite experience is preferred
