The RN Case Manager is responsible for coordinating comprehensive participant care in collaboration with an interdisciplinary team at St. Paul's PACE program. This innovative program provides coordinated medical care for individuals 55 years or older with chronic illness or disabilities. The nurse serves as a key clinical liaison between participants, families, and healthcare providers to promote safe and effective care.

Requirements

  • Current California Registered Nurses License
  • CPR certification
  • Minimum one year's experience in working with the frail or elderly population
  • IV Certification preferred
  • Valid driver's license and reliable transportation
