Role OverviewThe RN Case Manager provides discharge planning and utilization review activities for patients. They assess patients' physical, social, and mental wellness needs and develop comprehensive discharge plans. The RN Case Manager works within an interdisciplinary team and coordinates services with post-acute care providers and community-based organizations.

The RN Case Manager will address patients' special needs, assess their physical and mental wellness needs, and develop discharge plans. They will also monitor patients' progress, communicate with the interdisciplinary team, and coordinate services with post-acute care providers.

Requirements
- Registered Professional Nurse currently licensed in NYS
- Experience in Utilization Management (InterQual and/or MCG), Performance/Quality Improvement and D/C Planning
- Critical thinking, ability to multitask and problem solve
- Compassionate with teamwork skills
- Excellent verbal and written/electronic communication skills
