Role OverviewThe Hospice Case Manager delivers professional end-of-life care and services under the leadership of the Director of Hospice. They review patient and family needs and develop a plan of care with goals mutually set by the IDG team and the patient and/or caregiver.
What You Will Do
The Hospice Case Manager completes assessments, develops plans of care, provides emotional support and education, assists with daily living activities, and ensures compliance with established infection control procedures.
Why It Might Be a Fit
The ideal candidate is self-directed, able to work with minimal supervision, and has the ability to organize and prioritize workload independently. They should have one to two years of nursing experience, preferably in hospice, med-surge, oncology, ICU, or ED.
Requirements
- Associate's degree in nursing
- 1-2 years of nursing experience
- Current licensure to practice professional nursing in the state
- Basic Life Support certification
- Valid Driver's License
- Minimum automobile insurance
