Role OverviewLegacy Hospice is seeking a Full Time Hospice RN Case Manager (RNCM) to join our dedicated nursing team providing exceptional end-of-life care throughout the community. As a valued member of our interdisciplinary care team, you’ll help ensure every patient receives The Best Care Possible — care grounded in compassion, clinical excellence, and connection.
What You Will Do
Deliver hands-on hospice nursing care, coordinate services, and guide patients and families through every step of their hospice journey. Empower patients and families through education, encouraging their active participation in creating personalized care plans that align with their goals.

Requirements
- Hold current unencumbered license as a Registered Nurse.
- Minimum one (1) year experience as an RN in a medical surgical/acute care setting.
- Must be computer proficient in typing and various programs, including background in EMR.
- Possess and maintains current CPR certification if required by state.
- Current driver's license, vehicle insurance, and access to dependable transportation.
