Role OverviewThe RN Case Manager will evaluate patients' medical, psychosocial, and discharge needs, develop and implement individualized care plans, and coordinate safe and appropriate discharge plans. The role requires a bachelor's degree in a related field, at least one year of experience in case management, discharge planning, or nursing management, and a RN license. Preferred qualifications include a minimum of 5 years of acute care experience, CCM certification, and knowledge of Milliman Criteria and InterQual Criteria.
