Role OverviewPerforms comprehensive nursing assessments, integrates data, and develops plans of care. Maintains annual HHRG values and refers to community resources as needed.
What You Will Do
Evaluates patient needs, initiates and implements plans of care, and documents patient progress. Participates in inter-disciplinary case conferences and instructs Home Health Aides.
Why It Might Be a Fit
Requires successful passage of VNA pre-hire medication test, current RN license in Mass, and minimum one year of nursing experience. Preferred qualifications include Home care and IV experience, computer skills, and BSN.
